Patients are responsible for verifying the services covered by your insurance plan and the applied deductibles and co-pays.

For all HMO plans, it is the patient’s responsibility to name a primary care physician from our practice with the insurance plan prior to the date of the initial visit.

 

Primary Care Partnership participates and will direct bill the following insurances:

MA Blue Cross Blue Shield- all plans accepted except MABC Medicare HMO Plans

Blue Cross Blue Shield out of state plans - all plans that are part of the Blue Cross Blue Card network and allow claims to be submitted to the local Blue Cross plan. (Note: this excludes Blue Chip of RI)

Harvard Pilgrim Healthcare until 12/30/2010. Please see News.

Patients may elect to join the practice and pay out of pocket. Many insurance companies have out of network benefits. We will provide a copy of a detailed encounter form to submit for personal reimbursement. Please verify out of network coverage with your insurance company.

Mass Commonwealth Choice and Commonwealth Care Plans

Dr. Hood will continue to care for existing patients only whose insurance changes to one of these products.

Under the Massachusetts Health Care Reform Law, insurance companies now offer specific insurance plans for affordable health insurance for uninsured individuals. These plans are offered under the name of Commonwealth Choice. Each insurance company has instituted Commonwealth Choice plans and selected the providers who may accept the plans. Low income individuals may be eligible for the subsidized Commonwealth Care plans. Before selecting a plan, please verify that Dr. Hood is a primary care provider for the plan. Information is available at www.mass.gov/connector. The Customer Service Center numbers are Commonwealth Choice 1-877-213-8163 and Commonwealth Care 1-877-623-7773.

Below is a brief list of the plans we are aware of at this time.

Mass Blue Cross Blue Shield: Dr. Hood is an enhanced tier primary care provider for all Commonwealth choice plans except ESSENTIAL BLUE YA. This plan is for young adults age 19 to 26. Dr. Hood is a primary care provider in the standard tier for the YA plan and the member would pay higher out of pocket copay and deductibles.

Tufts Healh Plan: Dr. Hood is NOT in the network of primary care providers for the SELECT Commonwealth Choice plans for Tufts Health plans.

Mass Health/ Medicaid: Dr. Hood is currently not accepting new patients for Mass Health but will maintain an existing patient in the practice who switches into a Mass Health product. For eligible low income individuals, the Commonwealth plans for Mass Health are called Commonwealth Care and are offered through several companies. Boston Healthnet Commonwealth Care is the only plan for which Dr. Hood is a network provider.
