First Visit (New Patients)

• Send Authorization to Release Medical Records to current primary care physician
• Complete our New Patient Questionnaire
• Review and sign Patient/Provider Understanding
• Review the HIPAA Notice of Privacy Rights and Practices and sign the separate HIPAA Acknowledgment Form

Every Visit

• Bring all medicines you are currently taking; be sure to have dosage information
• Be familiar with services covered by your insurance and prepared to provide payment at the time of your visit for uncovered services - cash, check or credit cards accepted
• If having an EKG, avoid putting lotion on your body (interferes with reading)
• Bring results of any recent lab tests or imaging done outside PCP
• Arrive early for your appointment
• Bring any necessary forms to be filled out
